Types of Service
- Individual Service
- "Individual Service" is the plan where a Covered Member has enrolled for the individual option and the enrollee is the only person convered for the services described in Section 2 in Terms of Service.
- Family Service
- "Family Service" is the plan where a Covered Member has enrolled for the family option and the person(s) eligible for the services described in Section 2, in Terms of Service, shall be the Covered Member; spouse or Domestic Partner; children under the age of 21 who live with the Covered Member; and children under the age of 24 who are full-time students.
All orders are final. If subscriber chooses to cancel during the term of service, payment will not be pro-rated. However, cancelled service will continue until the renewal date, and at that time the service will not be renewed.
